COMPLAINT FOR CUSTODY
Use this form if you do not already have a case in this Court.  You can file this form if:

1) You are married but separated, and have not yet filed for divorce; or
2) You are a parent, relative, or friend of a child of which you are seeking custody.

CONFIDENTIAL INFORMATION

MOTION FOR CHANGE OF CUSTODY
Use this form if you are seeking custody of a child in a case already existing in this Court.  You must already be a party in this case to use this form.

MOTION TO ESTABLISH/MODIFY VISITATION
Use this form if you are seeking establishment of a visitation order or modification of a previously ordered visitation schedule in an already existing case in this Court.

MOTION FOR CONTEMPT
Use this form if you desire for this court to find a party of an already existing case in this Court in contempt for failure to obey with the orders issued in this Court.

MOTION TO MODIFY CHILD SUPPORT
Use this form if you are seeking a modification of a child support obligation previously ordered by this court.

MOTION TO MODIFY PRIOR COURT ORDER
Use this form if you are seeking a modification of an order previously issued by this Court.

MOTION TO ESTABLISH GRANDPARENT VISITATION
Use this form if you desire for this court to establish an order of visitation of your grandchild.
